Initial EHR Certification Bodies Named

Key step in national initiative toward adoption of electronic health records

The Certification Commission for Health Information Technology (CCHIT), Chicago, Ill. and the Drummond Group Inc. (DGI), Austin, Texas, were named today by the Office of the National Coordinator for Health Information Technology (ONC) as the first technology review bodies that have been authorized to test and certify electronic health record (EHR) systems for compliance with the standards and certification criteria that were issued by the U.S. Department of Health and Human Services earlier this year.

Announcement of these ONC-Authorized Testing and Certification Bodies (ONC-ATCBs) means that EHR vendors can now begin to have their products certified as meeting criteria to support meaningful use, a key step in the national initiative to encourage adoption and effective use of EHRs by America’s health care providers.

“Less than two months following the issuance of final meaningful use rules, we have approved our initial ONC-ATCB certifiers.  EHR vendors can begin immediately to get their products certified.” said David Blumenthal, M.D., national coordinator for Health Information Technology.  This is a crucial step because it ensures that certified EHR products will be available to support the achievement of the required meaningful use objectives, that these products will be aligned with one another on key standards, and that doctors and hospitals can invest with confidence in these certified systems.”

Applications for additional ONC-ATCBs are also under review.

Certification of EHRs is part of a broad initiative undertaken by Congress and President Obama under the Health Information Technology for Economic and Clinical Health (HITECH) Act, which was part of the American Recovery and Reinvestment Act (ARRA) of 2009.  HITECH created new incentive payment programs to help health providers as they transition from paper-based medical records to EHRs.  Incentive payments totaling as much as $27 billion may be made under the program.  Individual physicians and other eligible professionals can receive up to $44,000 through Medicare and almost $64,000 through Medicaid.  Hospitals can receive millions.

To qualify for the incentive payments, providers must not only adopt, but also demonstrate meaningful use of, certified EHR systems.  The law envisions that defined meaningful use requirements will help ensure that the patient and provider benefits of EHRs are realized.  Initial meaningful use criteria were defined in a final rule issued by the Centers for Medicare & Medicaid Services (CMS) on July 28.

In addition to the CMS rule, ONC also issued standards and certification criteria for EHRs on July 28, aimed at ensuring that EHR systems will support the specific tasks required under meaningful use.  Also, through regulations issued on June 24, ONC created a system by which technology review organizations could also qualify as ONC- ATCBs that will certify EHR products as meeting the requirements necessary for meaningful use.

With the initial two ONC-ATCBs now named, EHR vendors can apply to them for certification of their products.  By purchasing certified products, providers will have assurance that the products will support achievement of the meaningful use objectives.

“Multiple steps are underway to carry out the intent of Congress in supporting rapid and effective adoption of EHRs throughout our health care system,” Dr. Blumenthal said.  “The naming of initial ONC-ATCBs is one important step.  Actual certification of multiple vendors’ systems by the ONC-ATCBs is an important next step.  CMS is also working to create an online system for providers to register and attest for the EHR incentive programs. The first incentive payments are targeted to be made in May 2011.  Meanwhile, ONC is also carrying out new programs of technical assistance and training, especially for smaller hospitals and physician practices.”

Dr. Blumenthal said the Health IT initiative “is on an aggressive schedule to meet the urgent targets set by Congress and the President toward realizing the quality and safety improvements that we can achieve through health information technology.”

More Doctors, Hospitals Using Online Platforms for EHRs, Other Tasks

A growing number of physicians and hospitals are using online platforms and teleconferencing technology to communicate with patients, prescribe medication and share electronic health records with other health care providers, HealthLeaders Media reports.

In the past, health care facilities seeking to exchange medical information would need to purchase the same health IT systems, which could lead to disputes over cost and implementation.

However, online platforms often provide a non-vendor-specific way to connect with patients and collaborate with other medical practices.

In addition, Internet-based EHR systems, like our Ingenix CareTracker, allow health care providers to avoid the challenges associated with on-site data storage systems, such as server maintenance, information backup and data restoration.

Another benefit driving the increased use of Web-based tools is the ability to better connect with patients.

For example, secure e-mail portals can improve physicians’ efficiency and workflow by providing them with more flexibility to communicate with patients about non-urgent matters. New in CareTracker: Krames Patient Content

In the latest release, Ingenix CareTracker has partnered with Krames to provide practices with an extensive library of patient education material. Additionally, the Patient Education application from CareTracker EHR has been added to the Setup tab in the Practice Management Administration module. The application allows practices to upload and manage their own patient education materials.

Ingenix CareTracker PM & EMR v7.0 Released: Guaranteed to meet ARRA ‘meaningful use’ criteria

We are pleased to announce that CareTracker Version 7.0 has been released. There are many facets to this release, with the most critical being our guarantee that CareTracker meet the ARRA “meaningful use” criteria. CareTracker PM: New Features

NEW PRODUCT: HealthTracker
HealthTracker is a secure web-based patient portal that allows patients to track and manage their personal health information online. In HealthTracker, patients can:

In this release, Ingenix CareTracker has started the process of implementing a new Intelligence module. The Intelligence module is an optional add-on feature that provides access to a web-based version of the Cognos 8 Business Intelligence and Reporting tool. The Intelligence module will include both reporting dashboard features:

  • Reporting gives you access to a complete list of self-serve report types.
  • Dashboards communicate complex information quickly by translating information into visually rich presentations using gauges, maps, charts and other graphical elements.

CareTracker EMR: New Features

NEW FEATURE: E&M Evaluator
A feature that has been requested by many of our clients is finally here. The new E&M Evaluator application helps identify the most appropriate E&M procedure (CPT) code to use when charging for office visits and consultations. The code is calculated based on the information you document during the visit. Additionally, the application helps you apply either the 1995 or the 1997 E&M Documentation Guidelines issued by the American Medical Association (AMA) and the Centers for Medicare and Medicaid Services (CMS), to identify the correct code for the level of service provided. The ability to select the most appropriate code prevents under coding services you provide, improves payment by payers and reduces denials
due to insufficient documentation.

The application enables you to calculate the appropriate E&M CPT code using the three different methods listed below.

  • Chart Based - The application reviews the data recorded for the patient instantly and suggests an E&M CPT code that is compliant with Medicare’s Evaluation and Management (E&M) guidelines. Note: You must use a structured template for chart based calculation. Free text elements are not recognized.
  • Manual - The application enables you to calculate the E&M CPT code based on the selections made on the key components, including history, examination and medical
    decision making.
  • Time - The application enables you to calculate the E&M CPT code based on the total time spent with the patient when 50% or more of your visit was spent counseling the patient. The E&M Evaluator application is accessible by clicking the E&M Evaluator button on the Clinical toolbar.

21 percent cut to physicians who treat Medicare patients starts in days

March 30th, 2010

If you’ve not heard, physicians are potentially facing a 21.2% CMS fee schedule reduction. This decrease was already delayed, and is set to occur on April 1, 2010. The good news is, the House approved yet another delay in this reduction that would move the effective date to May 1, 2010. The bad news is that unfortunately Congress just recessed for 2 weeks without passing the bill that would in fact extend the decrease.

In response to this failure to take action (by Congress), the Centers for Medicare and Medicaid Services (CMS) has announced a 10 business day freeze on physician Medicare claims starting April 1, 2010, anticipating that Senate Democrats will extend the date for the fee cuts when they return on April 12.

Known Pitfalls and Proven Methods for a Successful EMR Implementation

EMR implementation is a hot topic these days, and each of our medical billing service clients have the opportunity to work with our integrated CareTracker PM & EMR solution (will also qualify you for the EMR incentives). With each passing day, EMR implementations continue to rise but one of the most important steps in launching your EMR is the first step - choosing the right implementation process.

The (linked) article below does a great job of discussing the known pitfalls and proven methods for successful EMR implementation. Many of the concepts in this article are key in our own EMR implementation process at HST. We are working closely with our EMR partner (and experts), Ingenix, to reduce the cost of implementation and make this transition to EMR as easy as possible. What we do know is that when you implement an EMR you will have to change your current workflows. This “change management” component of an EMR implementation is the key to success. Setting appropriate expectations (on both sides) up front during the process is important and will set yourself up for success during the implementation, training, and go live.

For Small-Practice Physicians, HIT Benefits Now Are Within Reach

Cost and complexity have been formidable barriers to small practices, particularly for their small business owners who already operate on thin margins. Perhaps what physicians have feared the most when considering HIT implementation is the potential of disrupting the operations of the practice and, ultimately, the impact on patient care.

Fortunately, recent trends may signal a sea change. Innovation on the part of HIT vendors has begun to reduce the costs of billing and electronic health record (EHR) systems.
